I have a question about how to fill out Additional Fam Info IMM 5406 form.
I am Divorced. The form asks for my spouses name DOB and Address Just below there is an area to sign if I dont have a spouse common law partner ex-spouse or former common law partner. I obviously can't sign this section but I am wondering if I am supposed to fill in her information on the spouse line. And if I am to fill in her info I dont know her address and marital status, I think she may have remarried
In your position, I would fill in what details you do know and then just write "Unknown" in the places that you don't have info for. I really am not sure of the legal position of providing information for an ex-spouse to a foreign government. After all, they are no longer related to you in any way and might see it as a violation of privacy.
I am divorced too. I was confused about the same question.
Of course, on the first form (Generic), I mentioned that I was divorced and provided details, on the other form, for that question I called CIC and they advised that i should leave it blank and not mention anything. In the cover letter, you need to put down an explanation and that is all.
I put N/A for spouse's name, and in place of my signature, I wrote "see attached explanation." Then I wrote a statement saying that I couldn't sign the form because I have an ex-spouse, provided the name, DOB, and contact information for my ex-spouse, wrote an edited statement saying that I did not have a current spouse/common-law partner, and signed it
