I just got a letter telling me I was approved. I have a few questions though..

So I just received a letter in the mail saying the processing of my application was complete and they need a few things in order to get my proof of perm. residency. A few questions just to be safe...
* Do I send it to the address on the letter? The Immigration Section at 1251 Avenue of the Americas?
* I'm from the USA. Do I need to photocopy *every* page of my passport? And not sending my actual passport? USA is a visa exempt country.
* The letter says to submit the Right of Permanent Residence Fee (RPRF), which we already paid. Should we send a copy of the receipt?
* We send 4 photos, right? 1 in one envelope with my name on the back of the photo, 3 others in a separate envelope with no writing?
* Is there a name for the type of photo they need? Should I ask for a perm. residency photo? Passport photo?
Someone else can help with some of the questions, but I do know that you need to send a copy of *every* page of your passport, even the blank ones. Send a copy of the receipt, and for the photos, your PPR letter should explain, but I think it's passport photos, and the info they want (I think it's eye color and something else?) goes on a separate paper, with the photos together.
