Hello,
I did a search in this forum on how to notify a legal name change to CIC but could not find a relevant answer. All I could find was how to notify address change for which there is an online form. I got the medical request form from CIC with my daughter's old name. So, I sent out an email to CIC last week asking them to resend it with new name. Also, I provided scanned copies of her new birth certificate in an email as a supporting document. So far, I did not receive any communication from them and the doctor's appointment date is getting near. Is there an alternate way to approach this issue? I was just thinking to download an empty Medical request form and take that to the doctor but the original document from CIC has an IMEI number which I cannot just copy paste in the new form.
Can anyone please let me know what you did in this situation?
