How to mention previous visa refusal?

And yes your father can reapply anytime (after working on the refusal reasons), unless otherwise stated in refusal letter.

Then you might want to do it soon.
Also, make sure to include what was missing and caused the refusal and add the same in an explanation letter (that you can upload in Client Info)

Shall we include the refusal reason in covering letter or add one more letter just to state previous refusal and its clarification??

Just one letter would be fine. Put all the info in one letter and attach any other supporting doc (that are not attached in the rest of the application) in one PDF and upload in the "Client Info"
