The link I gave you is a form to contact your visa office. The steps on how to use it are right there. You click on the question marks next to each field.
You explain your situation in the field called "Your enquiry". The visa office will reply to you to explain how to proceed.
Now that I review your situation again, it seems you haven't started your permanent residence application yet, right? If that's the case, you don't actually need to do this. Just file your PR application as single and include a letter of explanation.

Hi bellaluna it's me again, I already download almost of the requirements and i want to ask if the parent's consent need also to attach or just the explanation letter that we're not together anymore.
