how to apply for FSW directly without agency ?

Hi friends..i wish to apply directly for the FSW without any agency help. So where do i get the forms and stuffs for applying? can someone guide me plz?

Some of the forms may not apply to you - or you may have to complete all of them. You'll have to look at each one and decide if it applies to you.
