To those who already applied for work permit extension, kindly help me regarding this one: ( under live -in caregiver program)

6. Duration of expected employment from - to

For example: Date of application is Feb 6, 2013
Work permit expiration is June 6, 2013
will ask for 12 months extension
what will I write from - to

The from date would be the application date which is Feb 6th 2013 and the to date would be 12 months later.

Note: can i leave it blank? as in my job offer letter duration has not been mentioned since I will have a permanent job and I am also a provincial nominee.

Moreover, in my job offer letter the expected date to start is March 2015 or upon the working visa approval has mentioned. but there is no end of contract stated.

Need help .

I know this is a very old question, I am just replying for those who may have the same issue as this was left unanswered.

Please see my opinion below:

I don't believe you will be able to validate the page if you leave that section blank. Based on Guide 5487 - Applying for a Work Permit outside Canada: Question 5 "Note: A work permit cannot be issued for a duration that is longer than the validity of your passport." What I did in my case was to use my passport expiry date as the End/To date for duration of expected employment.
