How often is PR rejected after RPRF request

Wondering if anyone has ever heard of application being rejected after RPRF request?
And therefore the money had to be refunded?
Does this happen often?

Not often even I haven't heard , unless security clearance create any issues.

its not usual to reject, but from what i saw in my last GCMS (generated a day before PPR), there is a QA kind of process they check everything once.
