How can i add 6th dependent on the IMM0008 generic form? It says only 5 ppl can be added but including myself we are 6

How can I add 6th dependent on IMM0008 generic form as we are 6 ppl including myself as the principal applicant along with my spouse and 4 dependent children. But it says only 5 ppl can be added in the form. My lawyer said I should provide the information of the 6th dependent on a separate sheet but what to do with the question #3 of the form where I have mentioned 6 ppl as the form is not getting validated because only 5 entries are made. Plz guide
