HELP with MEDICALS forms AND which DMP is good... pleaseee... thanks

The medical forms which I get in my email was having only one form for me that is section A on which they have put the photo of primary applicant.

Question 1)
Section A : Form IMM 1017
Section BCDE : Form IMM 5419 (WHERE DO I GET THIS FORM ) ???????

Question 2
Also, does anyone KNOWS the best and reliable doctors in TORONTO ????? Please help me

Question 3
I just need my 2 Canadian passport size photo and PASSPORT as a proof of my ID and money to see a DOCTOR right ?????

I just went to do mine today:

1-I only printed the form in the e-mail. The doctor should have the appropriate forms needed - I presume.

2-I went to someone near Bathurst x College. Got an appointment in 2 days, and the whole thing took about 2 hours. I just worked off the list of doctors provided by CIC bottom-up to find the closest doctor, assuming that doctors at the top would be the most booked.

3-Yes, although she seemed to be surprised I brought "only" 2 photos. But seemingly, that was not an issue.

under the age of 11 years if the medical history or clinical examination of the child indicates there is a
need, or if there is evidence of tuberculosis in the family."

If any of the immediate family being examined have either had or suspected of having TB, then a child under 11 will require an x-ray. So, for example during your x-ray show a lesion on the x-ray, then the child will require an x-ray as well.
