GCMS consent form for privacy act ?

Hi guys,

I want to order GCMS notes for me. I live in Canada, so I think I can go for " Privacy Act ". When I'm filling up the application online, in the last page it says attach supporting documents ( consent form, any supporting document etc .. ). So, I downloaded the consent form, but that clearly says that " By signing this form, you're allowing your designated representative right to .... ".. Do I have to put the same form ? as I'm requesting information for myself . If not, then which consent form I should sign and upload ?
