Do we need to fill separate consent form for spouse while ordering gcms notes from cic website. Though there is a section on spouse in the form itself but the top of form reads as 'separate consent form for each individual in the file'.
I am getting a mixed opinion from people and I need some one who has gone through the process or ordering notes from cic website. Do i need to fill two 5744E form or just one. I dont want to be in situation where i will only receive half information about my file and that too after waiting for 30-40 daya.
PS - I have a relative in canada to whom i will designate.
