Good improvement, GCMS/Physical notes application is now available through website. Much easier then with the form before.
https://atip-aiprp.apps.gc.ca/atip/
I guess bad news to all those GCMS websites/agents
There is no change in requirement to be residing in Canada before you can make a request. Just a different format but no real change and companies will still be required by those without Canadian connections.

Isn't my courtesy enough? you also want to know my sources?
I got the link as an automatic reply when submitting GCMS request through email. They will stop accepting email application from middle May.
I certainly appreciate your posting the link, because it is a much faster process. Thank you.
However since the topic is privacy, the prudent thing is to verify sources before filling out ones private information into an online form.

Thanks Pippin. Did you ever encounter any issues ? I have a friend in Canada so should I go ahead and provide his address to order my notes ? Please advise.
Never a problem. Have been SO impressed with the courtesy with which I was treated by the people working in this branch of the government. Outstanding service!

If you want to be absolutely sure, it doesn't hurt to include a consent form. The relevant bit:
Consent, if applicable:
- If you are making a request on behalf of someone else, you must include consent to release their information. The consent must be signed and dated by the person authorizing the disclosure of his/her information.
- It is also recommended that you include a consent for each individual listed in your file over the age of 18.
- If submitting your request to CIC, please use the Consent for an Access to Information and Personal Information Request form (IMM 5744).
