I remember the time I went for my medicals in Feb 2020 in Seattle. The place pretty much exclusively did medicals for prospective immigrants to Canada. The doctor there had setup a big world map where the immigrant candidates can place a pin marking their place of origin. India looked like a hedgehog.

In your opinion do a lot of FSW-O Indian citizens get a PR card and not live in Canada for an extended period though? I know 2 people who've done that and their PR renewal eligibility has expired.

Same thing is happening with IELTS in India. When I booked my CD test in October 2019, dates were available 2-3 days later but now they are not available for 2 months even though the test is conducted 2 times a day 7 days a week.

Canadian PR is a backup for Indians living in the US. Many don't wanna give up their high-paying job but their place in the US is also uncertain. So, Canada is plan B. For Indians applying from India, Canada is a gateway to the US. To be honest, most Indians leave Canada after some time. I will also do that.

I know a few who have done that too. They pretty much treated it as a backup in case they couldn't get their H1B+i140 done in the US in time (Masters graduates can work for 3 years post graduation on OPT ... so that's 3 h1b lottery attempts).
I also know a few who moved to Canada for permanency, path towards first world citizenship and freedom from visa restrictions. Among my acquaintances, I would say that it's a 50-50 split.
