procedure yan ng pre june applicants, sa 1st aor confirmation of receipt lang, walang naka indicate na positive eligibility review, then sa 2nd aor nila same thing din confirmation of receipt, so malalaman lang nila if ok na docs nila when they have their MR na, sa atin naman all docs submitted na sa cio pa lang mismo, then comes sa vo, wala na tayong sinasubmit, sa prejune, kelangan pa nila magsubmit ng full docs sa vo and dun pa magstart ng eligibility review nila, so nangyayari walang work yung CIO, sa VO na lahat, unlike sa atin

yep 2 days isend na nila, kaso 2 days din yung medical, 1st day laboratory then 2nd day yung physical exam, pede mo din irequest na same day isubmit, may member dito sa forum na kay dr santos sa cebu doc nagpa med, same day sinend sa cem results nya, before ka umuwi ng leyte wait mo na lang until masubmit yung results para di ka na pabalik balik if in case may additional tests, then ask mo na pede send na nila same day ng med nyo para kamo makauwi na kayo :)

Just want to share our experience on our Medical Exam last saturday. Hope it helps someone who are planning to do their medicals at the same clinic. :)

Clinic: Nationwide Health Systems, Inc. - MAKATI (Near the Indonesian Embassy)

Start Time: 9:30 am
Time Finished: 12:00 noon

Applicants:
2 Adults (Full Medical Test) and 1 Child (2 yrs old / Physical Check-up and photocopy of immunization record only)
NOTE: Bring the original immunization record


PROCEDURES:
1. Present the MR Letter along with Passport, Picture and Photocopy of passport biopage in the reception to get a form.
NOTE: Bring BLACK BALLPEN no other color is allowed to fill up the forms.

2. Fill up the forms and attach your pictures.
NOTE: May glue dun sa Table 1 para sa pag attach ng pics.

3. Submit the finished forms to TABLE 2 and wait for your name to be called.
NOTE: They will give you a number and 1/4 piece of paper checklist


4. Your number will be called by the cashier to pay the fees.

5. Your number will be called in TABLE 3 and your medical check up will start.
NOTE: There are 4 stations, you can see it in the checklist mentioned above.

6. After you finished your medical exam, give your number and checklist to the reception and you can go home and wait for a call if further test is requested.

Yung checklist eh parang guide lang siya ng nationwide staff na tapos ka na sa Station nila, dun sila pipirma. There are 4 Stations para matapos mo ang Medical Exam mo, the following are as follows:

1) Eye, height and weight STATION
2) Laboratory (Dyan ka kukuna ng Dugo at Urine)
3) Physical Examination (Dyan ka titingnan ng doctor / you will remove your clothes here)
4) X-ray

After mo madaan lahat ng Station na yan, dapat may pirma din yung 1/4 piece of paper checklist mo. Then pag may pirma na lahat ibigay mo na yun along with your ID number sa reception, then tapos ka na.

Thanks for the tips. Plan ko magmedical kami ng family ko on april 11 at NHS. Sabi ng iba matagal daw magbigay ng feedback. Ok na rin sa akin kahit ganun kasi medyo madami din pinagdaanan ang husband ko in terms of his health.
Baka magpa-further tests sila. Yung panganay ko ay 14 y.o. at bunso ay 11 y.o. Hinihingi pa ba ang immunization record? Nagbayad ba kayo ng additinal fee for the courier service ng Nationwide? per head how much po?

1) Immunization Records: Yung sister ko ng magpa-medical siya hiningan din siya ng immunization records, she is in Grade 6 at that time. But it was 3 years ago, so I think it is better na magdala na rin kayo ng immunization records.

2)Courier fee: Automatic nila itong sinasama sa fees eh ;D. Bale dalawang receipt ang ibibigay sayo, first yung sa Medical fees then yung receipt ng courier, which is 50 pesos per head.

3) Medical Fees: For our family, which is compose of 3 members. We paid 9,900 pesos and if I'am not mistaken the breakdown is as follows:

2 Adults x Php 4,000 ---------Php 8,000
1 (2yrs old kid) x Php 1,750 --Php 1,750
Courier fee (3 x Php 50) -----Php 150
TOTAL --------------------- Php 9,900
