I could be wrong, but I believe its related to the number of attempts.. in the old system many folks got their application returned due to quota being filled for their NOC.. so if they were applying for the second time they'd select FSW2.. if it's the third time, then FSW3... and so on..
Could be wrong.. it's just my educated guess

Absolutely true. They are so quick to reject files over flimsy mistakes that could have been easily corrected with a " document request" as against outright refusal.
this is the thing now that they refuse applications so easily. but if you think from a VO 's point of view, they have the backlog plus new incoming EE appication twice each month (which they are supposed to complete in 6 months). therefore, most of them are not bothered with requesting any documents. I had provided lots of documents which were not even requested for EE such as full passport with blank pages too. You never know who would get your file and what they are looking for in your application. So in my opinion its better to provide every document even if you think it's unnecessary.
