You don't need to send photos to Visa Office at this stage but you need photos only if medical center, where you are going to have your medical examination, doesn't have e-medical facility. Please ask medical center at the time of reservation if you need to bring photos for medical examination or not.
You have to send both passports, don't discard or lose the old passport.

Since this is a new application for another NOC, I would recommend to have fresh proof of funds attached.

The PR visa will be valid for 1 year from the date of medical or up to validity of the passport which ever comes first.

I think visa will expire after 12 months regardless of passport expiry. in other words, expired passport does not cancel stamped visa but you will have to show a valid new passport as travel document and old passport for visa verification
