Format for DOB affidavit

I'm filling my CEC in July. My parents are currently in Canad for the visit, and I need to get the affidavit for my Date of Birth proof. Anybody has format/template for such an affidavit? I'll be submitting School Leaving Certificate, 10th grade certificate + affidavit.

You can go to any desi lawyer and they usually have a format.

Just a note on 10th certificate, only submit 10th grade if you have your name, BOTH parent's name with DOB on the certificate.

Yes both School leaving and 10th grade shows DOB, Parents name, my name. I'm just trying to be safe. Do I need to submit notarized copies of these documents?

Depending on the lawyer you go to, he will / should stamp the copies of supporting docs (10th certificate) you provide with the affidavit. I just attached those docs.

So you did not attached the affidavit, just the SLC and 10th grade certificate? DId you notarized them?
