Hi there

My wife is sponsoring me using the OUTLAND application form.

I currently live in Canada.

I have done my medical examination from a certified
physician here in Canada.

He said he would send my results to the government.
He just gave me a receipt to send along with my application

However, I don't remember receiving a form IMM1017 that
is mentioned in the checklist form IMM5491E.

IMM1017 is used if the panel physician doesn't use eMedical.

If you received a report/receipt with a barcode on the top (your UMI#), that's all you need. Send in a copy of that. CIC links your medical results to your application with that UMI#.

So I just realised that I have 2 receipts. One for an x-ray and the other for the medical test. Neither of which have a barcode or UMI number.

Would these receipts be enough to include in my immigration as proof of my medical exam?

The panel physician's office should have given you a report/receipt that says its from eMedical. It should have your photo on it, and a barcode/UMI# at the top (upper right if I remember correctly). That's what needs to be included with your application. They can't link to the exam results through eMedical without it.

If they didn't provide that, hopefully you can go back and request it?
 
No...they definitely did not give me anything like that. They just told me that they will send my results to immigration office
