I am a bit confused about the medical examination and the form IMM 1017. It seems that it needs to be done by a physician within Canada, however my wife is in Japan. How can I get this medical examination done in Japan? Does she need to come here for the medical exam?
You need a "Panel Physician" (a dr. approved by CIC). Check the list above for those in Japan. Call and make an appointment and ask what type and how many photos to bring with you. AND, remember to get a copy of that medical to put in with your PR application package.
You need to request an "Up Front" medical and then you don't have to wait.
The receipt that is supplied by the panel physician may be in one of two different formats depending on the system that they used.
Just ensure that it's been correctly completed And signed before leaving the panel physician's office.

I've done my upfront medical for spousal sponsorship , the DMP gave me only the copy of upfront medical notification UMI numbers with barcode, so can send this copy of my upfront medical notification even though the doctor did not give me the copy of IMM 1017 B.

Yes, that's what happens when the DMP is better organized. Those that seem only to be paper-based get the IMM 1017B instead. Either way, that's the document that you send off with the application. TAKE A COPY FIRST!! (Just in case the application is lost in the post)
