Filling up PRTD form

Hi All,

I have been trying to fill up PRTD form, and have few concerns regarding that:

1) Shall I fill the form online or handwritten?
2) I tried to fill it online, but was not able to sign it online. In that case can I take a print of the filled-out form and then sign it manually? Will that be accepted?
3) How much time does it take to receive the PRTD form? I am filling it from India.
4) Is there anything else in particular that I must pay attention to while filling the form?

Please assist me, as its a bit urgent.
