Filling out forms - The online receipt for payment - help

I printed out my receipt after paying fees online, and I'm wondering about the "Payer Information" box

Do I put MY payment details? Or do I put my mailing address for my application. Under name, do I put my name or the person who I am sponsoring's name?
And "Client Date of Birth". Do I fill it in? Whose date of birth?

i put our mailing address (we have the same address) and my husbands birthdate... i am the sponsor and he is the applicant.

i think i also wrote that i was his sponsor.
