But in case you really want to quit then write a withdrawal letter (signed by both you and your spouse) and send it to CHC New Delhi after putting a file no. sticker on it that you must have received.

Also send the scanned copy of that letter by Case Specific Enquiry at following:

which all forms i need to send...? as i dont know the actual representative as xxx my actual agent and when you check status there is some other xxxxx name and some other address which i need to submit in d cancellation form...for example my agent is haribhai based in baroda and in status it shows amit shah based in ahemdabad...
