In the letter, the office has asked for Additional Family Information. However, they have asked for form IMM5645. When I looked up on cic website, the form 5645 Is for visitors and 5406 is for the Permanent Residency application. I am confused about which one to fill 5645 or 5406. Can someone help?

Additionally. i am also requested to resubmit IMM5669 (Travel History). Since my dependent child is under 18 years, should i leave the dependent child above 18 as blank or should I fill as "Not applicable"?

My medicals got expired in January. I got a request for medical but surprisingly only for myself. Never ordered GCMS.
