Okay... I rethought about why I did not fill this up.
This was my thought process - In many countries when a child is born - usually they give the name Baby Smith or Baby Walker and issue the birth certificate in the hospital on the same day - which is in fact the Name at Birth. After this they change their name to let's say - John Smith or Jack Walker which is what they use for all documents which becomes their legal name and the name at birth doesn't appear in any other documents. I know this because my daughter was named as Baby Mother's Surname in the hospital books but I added the correct legal name in her birth certificates and hospital records as well.
I used Canada Post Xpresspost - it's fast and comes with tracking. Use the regular mail address on the instructions letter. One-day delivery, two-day delivery or even a week to deliver - it's all roughly the same in the grand scheme of things. I chose it just for the tracking benefits.
Feels to good to be back on this forum after 1.5 years, very helpful during PR and Spousal Sponsorship.

Just wondering, if someone can re-direct me to the thread which has information regarding supporting documents that needs to be provided along with the application.
For now , I have downloaded the following forms and working on it:
1. CIT002E
2. CIT007E
3. CIT117E
4. IMM5476
Please let me know, if any more forms are required as part of the application. I don't want my application to get returned due to some missing signature or form. Paper Applications are tricky
in this context, the app received means application delivered on the CIC office and there is no way to track your file until you get AOR. after AOR you will track your status on your CIC website. Thanks
I used fedex which has option to deliver next day. ups takes 2 days.
