Really? But plenty of ppl have changed their mailing address on the thread here from home
country to a friend / relative in the US. Merely to expedite receiving mail.

I'm just asking if I need to use the same application form or if there is some other form.

if you want to change ur mailing address, then there is a separate form. You can download that form their site.

I have emailed them scanned copy of signed request. They responded stating - it has been forwarded to concern department.

before they open my package and charge my CC as it's nearly a month since delivery.

My cc was charged just now. The packet was delivered on 21st February.

Credit Card Charged : 24-March-2017
Report Received by Me: 03-APR-2017
