On FBI Identification Record Request site, it says the cover letter and fingerprint card is needed. Now what is FD-258 form there? If I am adding Fingerprint card then do I need FD-258 form with fingerprints on it? I am bit unclear about it.

Now what is FD-258 form there is finger printing card. You can take a print out of the same on normal A4 paper and take finger prints on it

If you go to a professional services for Finger printing they will have the same form FD-258 in a card format.

FBI accepts both, the card and the A4 size paper

You can either print the form or use a pre-printed card, yes. The "cover letter" is really an application form that you download from the FBI website.
