Fathers info and birth certificate query

Need some help with a birth certificate issue. On my spouses birth certificate it only has her mother listed, and unknown for the father as at the time he did not what to be put on the birth certificate. Now he acknowledges he is her father and acts as so these days, but for when she has to list her mother and fathers details on IMM5540 Sponsor queestionnaire, should she list her fathers details even though he isn't on her birth certificate?

Any advice appreciated.

Yes, she should list his details and can just explain on a separate sheet about the birth cert.
