experts please help, new consent form for gcms notes got confused

hi all experts, As I am about to apply for gcms notes for the first time, here is the new updated gcms consent form.. and I got really confused.. please see this form and advise what and how to fill. I want to know information about my wife's file as I am the sponsor and she is the pa.

section A = is this PA or sponsor info?
section B = about whom?
section C (3) = whose information?
section C (4) = we don't have representative in our case and it's asking for signature or representative how to for this?

section D (5) = do I have to fill this?

She fills out Section a. You fill out section c. Then submit.

thanks for your reply but what about section B and section D is this vacant? and in section C it is telling sign of representative and we don't have any reps in our case.. who signs there PA or sponsor?

You are her "representative" or the person making the request. She is the applicant. She fills out section a, you (the representative) fill out section c.

thanks for help i just applied. I ticked the email option, so I believe email would be faster right? how many days for receiving? thanks again.

make sure the sponsor signs section C.

A lot of us got e-mails 30 days later to re-do the consent form because we didn't sign it. And have to wait another 30+ days.
