No worries, i think the idea of that form is to have either parent give consent to the other parent who intends to travel with the child. I do agree that it is confusing.
I'm asked to send a minor consent form from BLS. Our daughter will be travelling with me and my husband.

If parents are Canadians with surrendered Indian passport, you will need their surrender certificates. if not yet surrendered apply for it in parallel and include a copy of the receipt. If parents are Indian citizens, no SC is involved. The child would have no SC.
