No worries, i think the idea of that form is to have either parent give consent to the other parent who intends to travel with the child. I do agree that it is confusing.

I'm asked to send a minor consent form from BLS. Our daughter will be travelling with me and my husband.

So, I'm wondering how to fill the consent form??

We____________________________________________Father and Mother respectively of _______________________________________________. (Name of minor) hereby consent to our Son /daughter to travel with____________________.

what are we supposed to give in the space allocated for travel with???

Where did you submit, does Brampton accept, cause he’s 19, not a minor. So would they accept then

If parents are Canadians with surrendered Indian passport, you will need their surrender certificates. if not yet surrendered apply for it in parallel and include a copy of the receipt. If parents are Indian citizens, no SC is involved. The child would have no SC.

Hello I submitted in Edmonton BLS. I submitted on behalf of my spouse and my minor child. Spouse didn’t need to give consent.
