E-mail for sending Medical Results to CIO

Question for seniors.
I was apply for PR (Sydney NS) in April this year through SINP nomination.
Last week I finished upfront Medical exam and few days after I received Medical request from Immigration.
After upfront medical panel physician give me UMI number but on IMM 1017 E form (which I received few days later) I have IME number.
Today I was in my panel physician office and he give me new IME number (different from I have on IMM 1017 E form) and told me that I need send scan of Information sheet with new IME number to Immigration otherwise they will not be able to connect my medical exam report with my PR application.
My question is on which e-mail I need to send all this because I was never conntact them and I don't know where to send this documents ?

I got medical request yesterday and want to know if I can use the upfront medical that I got done in April 2015 for my work permit extention. Can anyone please suggest if I have to do a new one one now or the same should work?? The old one has a UMI # and in this request they send IMM1017E form which has completely different #s. Plz help!!

Yes you can.

Medical is the same in all cases. You just need to inform CIC to combine your upfront medical file with PR one. Good Luck!

I think you can not carry the medical more than six months as you did in April 2015 so its more than 6 months
