one form you have to write as applicant and other your husband applicant and spouse viceversa

one more question shall I add uae id number in national id document? and shall I attach it in the application as a proof of residence?

choosing permanent resident does not allow you to enter dates (from to) is that ok??

Say 'NA', Unless you have your home country NID.

national ID is yoru coutry ID
while u must have to attached ur residence visa clear page as proof of residence, but u can use EID as proof of residence (2nd document) but first is the visa and 2nd is EID

In generic application form it asks for uci number for spouse n dependents.....is it same as principle applicants uci number??? shall I write same ....or shud I leave it blank????
