Do the applicant, other than the main applicant need to fill IMM5645E family information form?

Hi,

My father is applying TRV, and including my mother in the application. Does my mother need to fill IMM5645E family information form? Although there is no place for the person being represented by a family member representative, I still wanted to confirm.

Yes your mother needs to fill in the IMM5645E.

As you are applying online. The person using their CIC account will have to be the appointed family representative
