Is it mandatory to list all my parent's siblings and their DOB's on this form ?

Yes, it is. It could be considered as misrepresentation/not declaring the complete facts if all siblings are not mentioned.
If your parents don't know the birth dates of their siblings then enter an '*' (asterisk) for each unknown date/month
