Sent email to Ottawa asking for IMM 1017 as DMP refusing to accept anything from Handbook or website.No reply from them, calling CIC everyday they keep disconnecting as all lines are busy.

Any other ideas ? Thanks for all your help

Keep calling CIC - you will get through when you least expect it :)

BTW, check your PM (top right of the pages on the forum click on 'inbox').
 
Ottawa replied now saying they will look into medical details how to deal with DMP as you guys are right we only need Appendix C.

Also second email saying my file transferred now to Visa office abroad.
