Discrepancies in date of birth

Hi everyone. Pls I need urgent responses from those who have gone pass the Express entry program. The date of birth on my international passport and declaration of age is different from the date of birth on my certificate. Pls do I stand a chance to be rejected by CIC. What's the way Forward. Thnks
