Decision Made before MEDICAL

I had applied Canada PR in August, 2006 from NEW DELHI VO. MY status was "In-process" with message "We have started processing your application on 24 Oct., 2006". In Oct., 2008, I received a letter from CIC informing therein that they are starting processing my application.

Now suddenly my status changed to "Decision-made" with the message "Decision has been made on your application. Office will contact you concerning the decision...."

What it means? Is my application rejected or MR or PPR can be required now????
