I'm in the same situation as you. I did upfront medical for my newborn. He has been added but after 2 weeks they sent medical request. I had already done the medical examination and was part of the required documents I submitted for him to be added.

What have you done so far? Any idea how to go about this?

Also, I did a raise a web form but no update

I found out that you need to contact your medical center and link your IME to your medical. You'll have to visit the medical center.

I sent someone there today, told told me to contact IRCC. They said it has been sent and they also gave me the information sheet which shows the medical was completed and was sent plus the date.

They did not say anything like linking. I even pressured them.

Oh, right. Let me tell you what they say when I make a visit to them. For now, I think web form is the only way, and I hope they see it.

2. Second one asking for IMM 5669 form to be filled, signed and submitted and police clearance certificate

My AOR is dec 7, 2020. Has anyone received any similar request? Why are they asking for PCC and IMM 5669? Any idea?
