So you lived together at least from Sept 2011 continuously to Sept 2012??

And you have proof from 5 years ago of the cohabitation?

Based on the general rule around common-law about long periods apart, you may find it difficult to get CIC to accept including your partner as common-law in your own application. 5 years of living separate is a long time.
