Clarification on Documents - IMM0008ENU_2D

I am trying to complete my documents for PR. I have few doubts while completing the docs.

1. Form 5669E (General form in Schedule A / Background declaration) Point 12. : It ask for list of Address. my query is:
Since I was ON and OFF from my home country do I have mention the details in sequence. example:
Date from ... date to Address xyz (Home country)
Date from ... date to Address ABC (Overseas)
Date from ... date to Address xyz (Home country)
Date from ... date to Address ABC1 (Overseas2)
Where Home Country address is same.
OR can I mention in One shot example:

Date from 1995... date to 2012 Address ABC (Home Country)
Date from .1998.. date to 1999 Address ABC (Overseas)
Date from .2005.. date to 2007 Address ABC (Overseas)

2. Rajesh-IMM0008ENU_2D.:

Point 13 for Spouse details :My Spouse is on Work Permit but not working(We have filled the tax returns without T4 doc. as SIN is present). Should the Status be: Vistor or Work ?
Point 10. I am in Canada, for Current country of residence what should be TO date; ?

Point#12 - you have to mention all the dates where you physically stayed.
Point#13 - Did she ever work on her WP?
Point#10 - You can write the TO date as the date on which you might be posting your application.

Yes, it should be in the above format.
OR can I mention in One shot example:

Date from 1995... date to 2012 Address ABC (Home Country)
