Change in last name

i have question regarding my change in last name. In National ID card and passport last name is mentioned but in all educational certificates no last name is included. Moreover i have WES certificate for educational credential assignment i have received a note of " The names on one or more of academic credentials submitted for evaluation differ from the name indicated above".
Please suggest what I should do? whether i apply for change in last name relative documents i.e. National ID card, passport and family certificate or any supportive document will be required without changing in my last name.
