Where it has all the different applications, is that the different stages it's gone through? It appears at one point my eligibility was passed

So you are saying that on the second page it says something like what i posted before but Eligibility - Review Required?

I think there is nothing to worry about in your case because if you noticed eligibility passed at one stage then it only means that they have requested another verification by an officer. Nothing to worry about.

I can't work it out, it looks like they say there is no date on my LOE, but I'm looking at it and there clearly is

Does the letter mention " XXXXX started his employment or started working with XXXXX company on XX, XX Date ?
