I did, but it got rejected because I didn't have a LMIA number. I even got an email from CIC where it says that I needed the LMIA or a second letter from CIC saying that I was approved for PR. Neither one I had, so had to apply for the last option: tourist visa. Sucks! But waiting for PR and hope it gets here soon. CIC recommended me back in September to get a tourist visa or a student visa and go back to school!! Now just the matter of waiting!
1: C
2: Your PGWP issue and expiry dates I think. Not 100% sure. I forgot the online form.
3: Mention your current work. To date should be blank so they know you are currently working. "Duration of expected employment" is N/A
4: I think all your post-secondary education should be mentioned.
5: I am not sure how to insert another row. There must be an option.
Also attach a letter of explanation saying you are applying for a BRIDGING OPEN work permit and you attached AOR of your PR application.

That's weird that CIC would say that..if you've been approved for PR, why would you need a BOWP? and why would you need an LMIA if you're trying to extend a PGWP...none of that makes sense. :-\
All the PGWP holders who applied for PR and are applying for the BOWP, can you please help me with these questions? What did you choose for these questions?
What would you like to do in Canada? ( I chose Work)
Work, Study, Move here
How long are you planning to stay in Canada?( I chose Temporarily more than 6 months)
Temporarily less than 6 months, Temporarily more than 6 months, Permanently
What is your current immigration status? ( I chose Worker)
Student, Worker, Temporary resident permit holder
But I need to confirm if these are right answers. Cuz after I finished this questionaire, I was not asked to submit copy of AOR or the PR document checklist. I am really lost.

Yeah. You put in your scores when completing the app but there is no section in the documents part to upload it so they sent a request and added another section under documents. I did wonder at the time why you upload a copy of everything else except that! :
