I currently have 7 years work experience as a Health Information Manager (NOC 12111). I noticed my job duties also include that of data administrator (NOC 21223).
After getting my reference letter from my work, which will clearly list duties from both NOCs, I want to apply to SINP with NOC 12111 then I apply to IRCC with NOC 21223 at the same time, using the same reference letter.

Your duties must match at least 70% of the NOC, so no letter should be able to support 2 NOCs. So in the database, if there are 8-9 duties listed you want to match at least 6. 3 duties is not even half.

I checked the duties of NOC 21223, which are 6 and my duties in my reference letter matched 4 of them. Also with NOC 12111 my duties matched all of them.

Well that is a big difference of 3. Pick your main NOC for Express Entry. Remember that through either EE or SINP, if your responsibilities are related to health then your NOC should be 12111. For SINP, your education must match the NOC. Would not suggest trying for different NOCs with the same letter as you can get into an issue of misrepresentation.
