Question 3 on the first page of the BOWP form,
I am applying one or more of the following,
1) work permit with the same employer
2) work permit with the new employer
3) restoration of temporary residence status as a worker
4) initial temporary residence permit or extention of temporary residence permit
I assume I should just choose 3) but the system asks me if it's new or same employer? Is it okay to choose 2) and 4)?

sorry, I meant 1 and 4,
I am working for the same employer but extending the permit under BWOP, someone said if we don't carefully choose here, the officer might give us a closed work permit.
is 1 and 4 okay in this scenario?
Or should I choose either 1 or 2?

Doesn't matter, it's an open work permit so employer does not matter at all. Lot of people chose option 2
and got the bowp approved.

You DON'T have to sign the form if you submit online.

Thank you. I am so nervous about this, as if I am not granted the BOWP my current work permit will have expired and so I can't re-apply.

also, I just realised that I did not scan the stamp pages of my passport. y visa expired today but I submitted my application 2 weeks ago. Can I add them to my application or should I just leave it?

Sometimes it's helpful to read application guides before applying.
Anyhow, if you have a filenumber or something that would make it easier for CIC to find
your application you could try emailing them.

Last time I applied for a WP was back in January and I was asked to scan all pages
with stamps or visas.

Good lord, how could I have missed that. Well maybe I'll be lucky and get the permit anyway, but I should probably start packing.
