birth certificate

I am trying for skilled migration, pls guide me on birth certificate as we don’t have and some one had suggested affidavit. If some one can share content to print on same .

Go to a Notary in your area. He/she knows how to prepare an affidavit. It should be attested by a Notary.
Also submit a certified copy of your high school certificate showing your birth date, place and father's name.
