I was asked to send passport, 2 photos and BIODATA form to CIC last week. But CIC did not attach the BIODATA form in their email as it should be. I emailed CIC back for it but get no reply.
Would anybody please email me one to Beijing780 @ gmail.com ? Please delete the blanks before and after the @

IMM 1017 (Client Biodata and summary form) is a GCMS generated form along with the medical instructions and not available on the CIC website ( I am sure you already are aware of this info).
You could try contacting the DMP in your area and get this form from them ( its available with them) which is labelled as IMM 1017 B (Upfront Client Biodata and summary form). Call beforehand to confirm if IMM 1017B would work for you.
