2- no you dont need police clearance certificate for that trip, only if you been to any country for more than 183 days.
1- checklist is on cic website with citizenship application forms.

updated,

you will get AOR (acknowledgement of receipt) which will include your file number, this will be done after the initial check about completion of your file

Quick one, pretty certain although my august application passport photos were stamped with studio/photographer details and date of photo I forgot to print my name on the back, chances of this not being returned?
