My wife received a letter from IRCC on Feb 8th saying that we have been granted a 30 day extension to submit the medical. But she gave birth to our daughter in January and still has pain in her lower abdomen due to C-section and the nearest medical center is about 450km from us. We don't want to fly and are planning to travel by road as it will also be easier for the baby. But we want to delay it by another 30 days.
Is it possible to ask for a 2nd extension of 30 days from IRCC? or Does that risk our application getting refused? The application is still being processed since 2020.

No your application should not be refused for this! I only asked for extension once and an updated imm1017 for my husband and took us 6 months for us to get our medicals (husband and dependant) done as we had to travel from Cuba to Russia to do it, and for international travel had to wait for a minor's passport. See my signature for dates, we now have PR! Send your Visa office a request for extension and explain why you are needing one. Mention covid-19 if you need to too. I used IMM1017 issued Sept 24, 2021 and June 28, 2021 for a Jan 24, 2022 appointment.
