Do you think we need to mark both of these Yes - if visitor visa to Canada denied?

d) have been refused refugee status an immigrant status or permanent resident status or visitor or temporary status to Canada or any country?
-> This one for sure is Yes

e) have been refused admission to or ordered to leave Canada or any other country?
-> This one I am not sure, as she never came to the border....it was visa that was denied..

A refused TRV means (d) would be a yes. You are right.

If she never showed up at the border then (e) is a no.
